A Seat at the Table: Culturally based cancer research among American Indians and Alaska Natives - Duration: 4:37.

Cancer has become a key research priority of the

Southcentral Foundation because there are many different types

of cancer where Alaska Native people have higher rates and

people are losing too many of their family members and there's

a desire to have the very best care available.

So, the community is really curious about, you know, what's

contributing to these higher rates and then really,

ultimately, wanting to impact cancer—not only for this

generation but for future generations.

Research in Indian Country often comes with a

negative connotation.

The assumption right now for many researchers is that

Native people will not participate in the collection of

biospecimen samples, and that is because for many Native people,

to have a fingernail or hair or blood or tissue sample,

we say "sample" in the research world,

but to Native people, it is a piece of them.

And it doesn't matter if it's in a petri dish or in a gel or in

a freezer, it's still part of them.

And so, for many Native people, thinking about participating in

this kind of research has been really hard because

we don't know what happens to those samples after

the research is completed.

I have worked on an NCI grant to do some bioethical work, to talk

about what does it mean to work with our Native populations and

to combine their data together to get a good sample size.

And what I learned from sitting down with elders,

tribal leaders, those who have experienced cancer, as they

talked about improving the disparities that exist in their

communities, is that it was all about collaborative partnership.

To have research that allows Native people to have research

that represents the true voices, the true narratives of what

they're experiencing, and to also have evidence on what works

and what doesn't work in our communities has been integral to

us looking at how do we undo the disparities

that currently exist.

The Southcentral Foundation research department has

conducted a large number of focus groups around the state,

around the area of cancer-related research and what

they would like.

They did want appropriate tribal oversight so that there was

somebody looking out for any stigmatization or misuse of the

specimens beyond the initial consent.

And also, people were very clear that they want to know the

results of this research—that the research actually get back

to the community, get back to providers so that they could

implement any new clinical efforts that may make sense, and

also that tribal leadership knew about the research findings

coming out of these efforts.

To be a practitioner of research in American Indian and

Alaska Native communities, it's more than community-based

participatory research, it's more than

tribal participatory research.

It means reframing our practices, and instead of

focusing on scientific rigor, we look at and we begin to

understand what it means to have cultural rigor, and that the two

are complementary to each other and not at odds.

It's not culture versus science.

It's culturally based science.

In the future, I hope to see that Native people are actually

experiencing some of the positive outcomes that result

from precision medicine,

the use of genetics and genomics information;

that Native people are brought to the table;

we are part of the conversations;

we are part of how to interpret that data;

we are part of shaping those research questions.

And that when the results of this precision medicine,

this amazing science that is happening, that it actually

impacts the people who are most disproportionately

affected by cancer disparities.

And in order to do that, we have to be at the table.
